You can also apply for a State Pension forecast by using an interactive version of the form. This form can be filled out online and then printed and posted to us. We have introduced a special feature that lets you save this form as you fill it in. This feature only works in Adobe Reader 5.1 and above. This means that you no longer have to complete the form in one session.

Both of these forms are in PDF format and come with notes that will help you fill them in and tell you where to send the completed form.

When you fill out the form you can say whether you would like to have your forecast sent to you or someone else.

Make sure you have your National Insurance (NI) number, your husband’s, wife’s or civil partner’s NI number (if relevant) ready when you fill it in.

Lines are open 8am to 8pm Monday to Friday and Saturday 9am to 1pm. Calls from BT landlines are charged at local rates.
